Job Description

To act as an advocate for parents and link them to resources in their community when deemed appropriate and when programmatic resources permit. Coordinate and implement family support services as identified through the Home and Community Based Waiver Service (HCBWS) plan process.  Utilize a strength-based family-centered approach to problem solving in collaboration with the Clinton County Coordinated Children’s Services Initiative (CCSI) and multiple community child and family providers.

Essential Duties & Responsibilities: 

  • Conduct outreach visits and engagement of families involved in HCBWS and CCSI
  • Assist families in the development of wraparound plans, resource acquisition and coordination of service plan implementation
  • Engage with service recipients in their homes and in the community
  • Encourage family participation in specific mental health programs/trainings and activities
  • Attend all relevant mental health training opportunities as approved
  • Coordinate and implement family support services as identified through the Home and Community Based Waiver Service plan process
  • Attend department meetings and trainings as required
  • Perform other duties as assigned within context of job description

Requirements

  • Parents or foster parents with experience raising a child with a psychiatric, behavioral or emotional disorder required.
  • Must receive clearance from NYS Register of Child Abuse and Maltreatment.
  • Excellent communication skills.
  • Flexibility in work hours/days.
  • Valid NYS driver’s license and reliable transportation.
  • Local travel within Clinton County.
  • Bachelor’s degree and or experience in related human services field.
  • Engage with consumers in their homes and in the community.
