Job Description

This position conducts health and recovery plan (HARP) assessments in order to determine eligibility for Home and Community Based Services (HCBS). This position will formulate plans of care and complete assessment outcomes.  The person will also assist in engaging individuals in the process of assessment, treatment planning, and potential referral to HCBS services.

Essential Duties & Responsibilities:

  • Conduct Brief Eligibility Assessments on HARP identified clients to determine HCBS eligibility
  • Complete Preliminary Plan of Care and submit to MCO
  • Complete Full Plan of Care on an initial and annual basis
  • Coordinate services with Managed Care Organization (MCO) and/or referral source, and HCBS providers as necessary
  • Regional travel to administer assessments and complete plans of care.
  • Assist BHSN programs with PSYCKES (statewide database system) initiative identifying HARP clients potentially not enrolled in Care Management services 
  • Provide education regarding HARP/HCBS services
  • Provide data for various reports including AHI Comprehensive Assessment
  • Enter information into the designated statewide data system
  • Ensure and track due dates for billing and compliance are met
  • Coordinate with Care Managers and MCO’s regarding service delivery on an ongoing basis
  • Coordinate services with HCBS providers
  • Assist with outreach and engagement activities
  • Follow policies and procedures outlined by program, federal, state and organizational regulations
  • Participate in required training
  • Participatory member of staff and supervisory meetings
  • Additional duties as assigned within scope of job description



  • Education Requirements:
  • A bachelor’s degree in any of the following: child & family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation therapy, rehabilitation, social work, sociology, or speech and hearing; OR
  • NYS licensure and current registration as a Registered Nurse and a bachelor’s degree; OR
  • A bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; OR
  • A Credentialed Alcoholism and Substance Abuse Counselor (CASAC).
  • Professional Work Experience Requirements (in addition to education requirements):
  • Two years of experience (a Master’s degree in a related field may be substituted for one year’s experience) in EITHER:
  • Providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse; OR
  • Linking persons who have been diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse to a broad range of services essential to successfully living in a community setting.
  • Certified Community Mental Health Assessor with Uniform Assessment System (UAS) required within 30 days of hire
  • Ability to work independently and collaboratively
  • Strong written communication skills
  • Ongoing maintenance of CPR/Safety training certification required
